The discounted price is currently $7.80 per copy plus plus applicable tax and shipping. Orders must be for 12 or more copies and need to include payment (check or major credit card) and a document verifying health service status and your IRS license number for tax exemption. The document must be a copy of the statement filed with a state or federal agency indicating health services or health education as a primary purpose of your group. Changing Bodies, Changing Lives is available at a discount to clinics, groups, and organizations licensed by the state to operate entirely or in part as counseling services, health clinics, or mental health clinics offering health education and information to teenagers, for distribution to their members or clients. The discounted price is $6.90 per copy, plus $1.40 per copy to cover shipping. A minimum of twelve copies must be ordered, with payment by money order or cashier's check. A copy of the state or federal license (or other document filed with the state or federal government indicating as one of the organizations purposes the teenager education function as described above) must be included with the order.
